Orthovisc is a prescription medication used as a viscoelastic supplement or a replacement for synovial (joint) fluid in human joints for osteoarthritis pain reduction. In knees with osteoarthritis, the joint fluid (called synovial fluid) can break down and not provide the cushioning your knee needs. The medication Orthovisc is a sterile and non-pyrogenic solution of hyaluronan contained in a single-use syringe. Hyaluronan is a natural complex sugar of the glycosaminoglycan family. The hyaluronan is extracted from rooster combs. This medication is indicated for the treatment of pain in osteoarthritis (OA) of the knee in patients who have failed to respond adequately to conservative non-pharmacological therapy or simple painkillers such as acetaminophen. Only medical professionals trained in accepted injection techniques for delivering agents into the knee joint should inject Orthovisc for the indicated use.

In a study (Efficacy and safety of intraarticular sodium hyaluronate in knee osteoarthritis, 2001), Brandt, Kenneth D. MD; Block, Joel A. MD; Michalski, Joseph P. MD; Moreland, Larry W. MD; Caldwell, Jacques R. MD; Lavin, Philip T. PhD ORTHOVISC Study Group, found that “sodium hyaluronate treatment is well tolerated and produces statistically and clinically significant improvement of symptoms in patients with mild to moderate knee osteoarthritis in whom pain in the contralateral knee is relatively modest.”

Orthovisc is available from Canada and Turkey manufactured by Anika Therapeutics. There is no generic form of Orthovisc available yet. Each mL of Orthovisc contains 15 mg of sodium hyaluronate. It is available in a disposable glass syringe delivering 2mL of sodium hyaluronate dissolved in physiological saline.

Stop taking Orthovisc and call your doctor at once if you have one of these serious side effects:

• Allergic reactions including hives; difficult breathing; swelling of your face, lips, tongue, or throat
• Bleeding
• Increased knee pain
• Signs of infection (warmth, swelling, redness) around your knee

Less serious side effects arthralgia (joint pain), stiffness or swelling, itching, numbness, tingling, headache, water retention, dizziness, back pain and swelling, pain, redness, or mild discomfort where the medicine was injected. Please call your doctor if you have any further concerns or questions.

Do not take the injection if you are allergic to are allergic to hyaluronan, are allergic to gram-positive bacteria proteins (ask your doctor if you are not sure), have known allergies to avian or avian-derived products (including eggs, feathers, or poultry), have a bleeding or blood clotting disorder such as haemophilia, have an infection in your knee or in the skin around your knee. This medication should not be used by anyone under 21 years of age. You should always tell your health care professional about any medications that you have been taking or are going to take including prescription and non-prescription drugs, vitamins and herbal supplements. Before taking Orthovisc injection, inform your doctor if you are pregnant or breastfeeding. It is not known whether Orthovisc will harm your unborn baby. Tell your doctor if you are pregnant or plan to become pregnant while using this medication. It is not known whether hyaluronan passes into breast milk or if it could harm your nursing baby. Tell your doctor if you are breast-feeding a baby.

Avoid jogging, strenuous activity, or high impact sports such as tennis for at least 48 hours after your injection. Also, avoid weight-bearing and standing for longer than 1 hour at a time. Do not concomitantly use disinfectants containing quaternary ammonium salts for skin preparation as hyaluronic acid can precipitate in their presence. The safety and effectiveness of the use of Orthovisc in joints other than the knee have not been established. The effectiveness of a single treatment cycle of fewer than 3 injections has not been established. Pain relief may not be seen until after the third injection. The safety and effectiveness have not been established for more than one course of treatment.

Orthovisc is given in an injection directly into the knee in a series of intra-articular injections one week apart for a total of three or four infections. (Source)
